Faculty Recording Agreement

Under section 504, Subpart E Post Secondary Education, of the 1973 Rehabilitation Act and the Americans with Disabilities Act, institutions of higher education must provide reasonable accommodations to a student's known disability and may not deny equal access to the institution’s programs, courses and activities. Tape recording lectures is a reasonable accommodation for students whose documentation calls for this accommodation.

Faculty have the right to require a student who uses a tape recorder to sign an agreement for tape recording and present the form to the instructor.

I further grant permission to Barry University to make the recording and/or transcription available to a student as an accommodation to a disability, subject to the condition that the transcription is limited to instructional use by this student and may not be copied, shared, distributed, or reproduced in any additional way without my specific consent.

It is understood and agreed that I retain full copyright in any recording or transcriptions of my classroom lectures and discussion and that this consent is given only for the limited purpose of accommodating a student with a disability as described above.

I acknowledge that I have given this consent voluntarily and that no compensation has been given or promised to me in exchange for this consent. I am of full age and have the right to contract in my own name. I have read this agreement and fully understand its contents. This release shall be binding upon me, my heirs, representatives and assigns.
