Required Health Information

All students residing in any/all university provided housing and/or international students attending Barry University on an F-1 or J-1 visa are required to provide the Student Health Center (SHC) with all of the proper immunization and insurance documentation listed below.

Failure to comply with the University health policy may negatively affect the student’s registration process and/or university housing arrangements.

Immunization

Medically documented proof of:

  • Adult tetanus vaccination within the past 10 years (Tdap preferred)
  • Two measles-mumps-rubella (MMR) vaccines (with the first dose at the age of 12 to 15 months or later and the second dose at age 4 to 6 years or later) or laboratory proof of immunity to measles, mumps, and rubella
  • Meningitis vaccination within the past five years (required if living in campus housing)

All students are strongly encouraged to provide a statement of good health and proof of Hepatitis B, Hepatitis A, Varicella (if no documented case of chicken pox), annual influenza vaccinations, and PPD (tuberculosis) screenings prior to attending Barry University. All required and recommended vaccinations are available at the SHC.

Note: Immunization records received by the SHC are entered into the University computer system for lifelong retrieval. Hard copies of vaccinations administered by the staff of the SHC are maintained for seven years in the SHC. The SHC assists all academic programs by providing those immunizations that are required for academic matriculation and clinical rotations.

Acceptable records of immunizations may be obtained from the following sources and must be presented to the Student Health Center prior to the first day of classes and/or moving into any/all university provided housing:

  • High school records
  • Personal immunization records (signed or stamped by a health care provider)
  • Health Department or physician records
  • Military documents or World Health Organization documents
  • Previous college or university immunization records (These records do not automatically transfer - you must request a copy.)

Insurance Requirements

For all students residing in university provided housing and/or attending class on and F1 or J1 visa

All students residing in any/all university provided housing and/or attending classes on an F1 or J1 visa, must provide proof of insurance and complete an insurance waiver through myBarry prior to checking in to university housing and/or prior to the first day of class, or the fee for the primary Barry-sponsored student insurance plan will be assessed to the student’s account. If a student insurance fee has been assessed to a student’s account due to lack of compliance with the University policy, such fees are irrevocable and will not be credited under any circumstance.

All students residing in university provided housing:

All students residing in any/all university provided housing (including campus owned apartments) will be charged a bi-annual (once in fall and once in spring) charge of $97 for the Student Health Center Supplemental Insurance plan unless otherwise waived. The Barry University Student Supplemental Plan is designed to ensure access to healthcare for students who have limited transportation to off-campus providers and/or whose primary insurance plan includes a high-deductible and limited access to specialty care.  Services covered by this plan include but are not limited to; $500.00 maximum coverage for wellness benefits, $200.00 coverage for age appropriate immunizations. Services provided at the Student Health Center only are covered at 100% including medications provided at the time of the visit. In addition, this plan also provides a $2,500.00 per semester benefit for out-patient primary care expenses. This is a limited duration policy with no inpatient hospitalization benefits (policy exclusions and limitations apply). Students may waive out of the supplemental plan if they meet the waiver requirements. Please note: students with private insurance must complete the primary insurance waiver and the supplemental waiver to be eligible to receive credit for the supplemental plan. By waiving the Supplemental Plan, the student is indicating that they would prefer to receive their primary care at an off campus provider. Basic health services such as self help and referral services are available for all students. Students requesting the primary student insurance will not be billed for the supplemental. All of the Student Health Center benefits within the supplemental plan are included in the primary plan.

Note: Students who receive a denied waiver for their primary plan will be billed a non-refundable fee for the Barry Student Insurance Plan.

All required health forms can be downloaded at www.barry.edu/health-services/. Waiver forms must be completed through myBarry.

Please make copies of all records and information you send to the Student Health Center for your personal files.
