Important Health Services Reminder
Student Health Center would like to remind parents that all students
residing in campus housing and students attending the university
on an F-1 or J-1 visa are required to have the following:
- Current health history with documentation of a tetanus diphtheria
vaccination within the last ten years. Students born after 1956
must have medical documentation of two measles vaccination or
a natural case of measles. (Note: It is strongly recommended that
all entering students receive a PPD, physical examination, and
a Meningitis vaccination, Hepatitis vaccinations and Varicella
vaccinations (if a natural case of chicken pox has not been documented).
- A signed medical authorization form.
- All forms must be completed and on file in the Student Health
Center.
- Proof of adequate medical insurance in South Florida by hard
wavier.
Many out-of- state and foreign insurance companies do not provide
adequate health care coverage in South Florida. If you do have private
insurance, you must complete the online waiver form at www.SRwaivers.com.
Failure to complete this waiver will result in a non-refundable
insurance billing fee on your student account. Please contact your
company’s member services to verify primary care coverage
in the South Florida region. For your convenience, Barry University
offers an affordable student health insurance plan to all undergraduates
enrolled in 9 and graduate students enrolled in 4 or more credits
per semester.
