What to Do Before Your Appointment

To help us provide you with the best quality of care, there are several things that you will need to do prior to your visit:

Bring a list of all of your previously prescribed and over the counter medications and other pertinent health information with you to your appointment.

Please report to the clinic 30 minutes prior to your appointment.

Approximately two days before your visit, you will receive a call to remind you of your appointment. If you are unable to make the appointment or need to reschedule, please contact the office location where you made the appointment.
