Student Health Services Requirements

All Students

  • To support student health and well-being, Barry University recommends that all students have health insurance that provides full benefits where they attend classes or participate in clinical rotations. The University offers a group health insurance plan through UnitedHealthcare Student Resources, which includes nationwide coverage and additional benefits for medical repatriation and evacuation while traveling abroad.

    Students in certain categories are automatically enrolled and billed at the start of each semester. They have three weeks to provide proof of comparable private coverage in Florida and request to opt out. After registration, students will receive an email from Barry’s student insurance partner, Academic Health Plans (Risk Management), once the Waiver/Enrollment site is open. If no waiver is submitted and approved during this period, charges will continue each semester the student is enrolled. Waivers must be resubmitted each Fall semester. Students who do not meet the automatic enrollment criteria are considered voluntary and are not eligible for this plan.

  • Fill out the Required Health Form.

  • If your academic program requires specific vaccinations or proof of insurance, please submit all information to your academic compliance personnel in addition to the Student Health Services.

  • Required Vaccines for On-Campus Residents

    Hepatitis B (Hep B)

    • Three (3) documented doses
    • OR acceptable laboratory evidence of immunity

    Measles, Mumps, Rubella (MMR)

    • Two (2) documented doses
    • First dose must be given on or after your 1st birthday
    • OR acceptable laboratory evidence of immunity

    Meningococcal Conjugate (MCV4)

    • One (1) dose given at age 16 or older

    Tetanus, Diphtheria, Pertussis (Tdap)

    • One (1) dose within the last 10 years

    International Commuter Students (Not Living On Campus)

    Must provide one of the following:

    • Two (2) documented MMR doses
      • First dose must be given on or after your 1st birthday
    • OR acceptable laboratory evidence of immunity

    Documentation Requirements

    Your records must include:

    • Full name and date of birth
    • Vaccine name
    • Dates administered
    • Healthcare provider or clinic information

     Incomplete or missing records will result in non-compliance.

    Important Information

    • Students who do not meet these requirements may be subject to housing restrictions.
    • Upload all required documents to the Student Health Portal

    Exemptions (Medical or Religious)

    • Contact Student Health Services to request an exemption form
    • If approved, an MMR titer is required
      During an outbreak, exempt students may be required to:
      • Temporarily leave campus housing
      • Not attend classes until risk has passed
    • Students are responsible for:
      • Alternative housing costs
      • Coordinating academic absences

    About These Vaccines (Quick Overview)

    • Hepatitis B: A vaccine-preventable liver infection spread through blood and body fluids that can become chronic and lead to serious liver disease.
    • MMR (Measles, Mumps, Rubella): Protects against highly contagious viral diseases. Measles can cause pneumonia or brain swelling, mumps can affect glands and lead to complications such as meningitis, and rubella can cause fever, rash, and joint symptoms.
    • Tdap (Tetanus, Diphtheria, Pertussis): Protects against serious bacterial infections—tetanus causes painful muscle stiffness, diphtheria can lead to breathing problems and heart complications, and pertussis causes severe coughing that can make it difficult to breathe.
    • Meningococcal (MCV4): Protects against meningococcal disease, a serious infection that can cause meningitis or bloodstream infections and can become life-threatening very quickly.

  • Serogroup B Meningococcalvaccine

    MenB-4C (Bexsero®, 2 dose series) MenB-FHbp (Trumenba®, 2 or 3 dose series)

    Varicella (Chickenpox)

    Varicella (Chickenpox) - Provide proof of two doses of Varivax. OR: Provide results of a blood test on a lab form verifying immunity to Chickenpox/Varicella. ***NOTE: All titers must include a lab report* Students in many academic health programs are required to provide proof of these vaccines and/or titer.

    Hepatitis A

    Hepatitis A, HPV, Polio, Other - In the boxes provided in this section you may also list any additional vaccines that were administered. These are not required.

    HPV (Gardasil or Cervarix)

    Polio (last date)

    Tuberculosis Screening: TB Skin Test by PPD (Mantoux)

    Tuberculosis Screening: A Tuberculosis Skin Test by PPD or Mantoux (within the last year) is required for students in many academic health programs.

    Chest X-ray (if positive PPD or lab)

New Students 

  • Download the Immunization Record form. Take to health care provider to be completed and signed. Go to https://barry.studenthealthportal.com and follow the instructions for entering your immunizations. You will have access to the portal after you have submitted your deposit to the University or you have registered for the first time at Barry University. 
  • The student health portal is designed to help you manage your immunization requirements quickly and easily. You can use this site to submit various online forms and track your own health history at Student Health Services. Immunizations must be entered into the portal prior to the first day of classes. If you have any difficulty with the portal, please email your records to healthservices@barry.edu or call Student Health Services at 305 899-3750 for assistance. 
  • Acceptable records of immunizations may be obtained from the following sources:
    • High School records
    • Personal shot records (signed or stamped by a healthcare provider)
    • Health Department or physician records
    • Military documents or World Health Organization documents
    • Previous college or university immunization records (these records do not automatically transfer, you must request a copy)

Please note: All paperwork must be entered into the portal prior to the first day of classes. Failure to provide documentation will delay the registration process for subsequent semesters and will impact your campus housing.

Please make duplicate copies of all records and information you send to Student Health Services for your personal files.

Getting Your Vaccines at Student Health Services

All vaccines are available at a reduced cost through Student Health Services (SHS). 

  • If you have the University’s student insurance (UnitedHealthcare Student Resources): Your vaccines are covered at 100% — no cost to you. 
  • If you have private insurance: You can still get your vaccines at SHS, but your plan may not cover the full cost. In some cases, it might be cheaper to go to an outside provider. If you choose SHS, the cost will be billed to your student account. We’ll submit a claim to your insurance, and if they pay, that amount will be credited back to your account.

Important: SHS cannot file claims with any Medicaid plans.

Immunization History Record 

  • To retrieve your current Immunization History Record once you have registered for classes and uploaded your immunization forms to the Student Health Portal please log onto http://barry.studenthealthportal.com/
  • Print an immunization record from the portal

If you would like your Immunization History Record officially stamped, please visit Student Health Services in Landon 104.
