VA Forms

  • Use this form to apply for educational assistance under the following benefit programs:

    • Post- 9/11 GI Bill chapter 33 of title 38, U.S. Code
    • Montgomery GI Bill (MGIB) chapter 30 of title 38, U.S. Code
    • Montgomery GI Bill - Selected Reserve (MGIB-SR) chapter 1606 of title 10, U.S. Code
    • Post-Vietnam Era Veterans' Educational Assistance Program (VEAP) chapter 32 of title 38, U.S. Code, or section 901 or section 903 of Public Law 96-342
  • Use this form to apply for educational assistance if your sponsor has transfer Post-9/11 GI Bill benefits once the DoD approves the Transfer of Entitlement (TOE).

  • Use this form when:

    • You’re changing schools,
    • You’re changing your educational, professional, vocational goal,
    • You left your program due to unsatisfactory attendance, progress, or conduct; and you're now reentering the same program,
    • You were receiving VA education benefits as a veteran and now wish to receive benefits while on active military duty, or
    • You have exhausted your Post-9/11 GI Bill benefits or will exhaust all benefits within the next 180 days and would like to apply for the Edith Nourse Rogers STEM Scholarship

    To be eligible for the Edith Nourse Rogers STEM Scholarship, you must meet all the requirements below.

    You:

    • Are using or recently used Post-9/11 GI Bill or Fry Scholarship benefits
    • Have used all your education benefits or are within 6 months of doing so. Check remaining benefits
    • Are enrolled in an undergraduate degree program for science, technology, engineering or math (STEM), or have already earned a STEM degree and are pursuing a teaching certification. See approved STEM programs
  • If you are a dependent (spouse or child) and this is your first time using the Fry Scholarship or DEA (Chapter 35) educational benefits.

  • If you are a dependent (spouse or child) and you have previously used the Fry Scholarship or DEA (Chapter 35) educational benefits and need to report the following:

    • You’re changing schools,
    • You’re changing your educational, professional, vocational goal,
    • You left your program due to unsatisfactory attendance, progress, or conduct; and you're now reentering the same program

University VA Forms

  • Students are responsible to submit a VA Certification Request each term after they have registered for the semester/term. You do not need to submit this if you do not wish to use VA Education Benefits for the registered semester/term.

    If this is your first term at Barry University, once you have registered, please submit your VA Certification Request and select Yes to the First Term at Barry University question. The form will navigate you to upload your supporting documents.
